Electronic medical records allow for 24 hour access to a patient’s health information. Therefore, we are in the process of converting all of our patient’s medical records from paper to a digital format (via DigiChart). Please be assured that every effort has been made by Carlos & Parnell, M.D., P.A. and digiChart to maintain the confidentiality of your health information during and after this conversion.

When you schedule your appointment you will be given a digiChart username and password, which will allow access to the patient portal. Upon entering the digiChart patient portal, you can enter information regarding your current health concerns, as well as your past medical, surgical, and family history. Your physician will use this information to facilitate and expedite your visit to the office.

It is imperative that you complete the digiChart patient portal prior to your office visit. If you are unable to complete the patient portal prior to your office visit, then you MUST ARRIVE TO THE OFFICE AT LEAST 30 MINUTES PRIOR TO YOUR SCHEDULED APPOINTMENT TIME in order to complete the patient portal via a computer kiosk in our office.
